Policies-Please initial below
*Proper attire is required to
participate in classes at the gym. Girls may wear a leotard or a one piece
bathing suit for gymnastics or shorts (no zippers) and shirt
for cheer and tumbling, no jeans or short tops.
* Boys may wear gym trunks and
a T-shirt.
* No jewelry is to be worn,
and hair must be tied back securely with a hair
elastic.
*For safety reasons, children
who do not have the proper attire will not be allowed to participate that day.
*Parents are always welcome to
stay and watch but must do so from the viewing area. Please do not talk to, or
otherwise distract your children during class. Any interruption could result in
serious injury. Children not in class must be kept in the viewing area
also under supervision
* Non-students may not be in
the gym area.
*No videotaping or flash
photography during classes is allowed; however, you can arrange to take
pictures of your child doing gymnastics when it is not a distraction to other
students, please check with management.
*It is best for your child to
attend her/his regularly scheduled class. Accounts will not be adjusted
or prorated for any classes missed as we have hired instructors to teach the
class whether or not your child attends. Make ups for illnesses can be
scheduled through the office and based on class availability. This is a
courtesy offered and make ups should be completed within 4 weeks of the missed
class.
* Payments may be made by
Cash, Personal check, Visa, or MasterCard, or direct pay monthly. Returned
checks will carry a $20.00 processing fee. NO REFUNDS OR CREDITS. Payment
is due monthly or by session before the first class. A late fee of $15
will be assessed after the first week of the month
or session. These policies help us keep our class size limits for the
advantage of all students.
*CANCELLATION notice must be
given to the office in writing two weeks prior to dropping from the
program. You will continue to be billed for the full tuition until notice
is given.

I understand that gymnastics
is a sport that involves height and rotation of the body; therefore, there are inherent
risks
involved. I hereby testify as to my Son/Daughter's sound
health of mind and body and I authorize Advanced Gymnastics and Xtreme cheer and its staff to seek medical treatment in
case of emergency. I attest that I have current and valid medical
insurance and will be financially responsible for any and all emergency and
medical expenses.
I have been informed and
recognize that gymnastics, cheer, and related programs of Advanced Gymnastics
& Xtreme Cheer can be inherently dangerous and
may cause injuries, possibly minor, serious, or catastrophic in nature,
paralysis and even death. I am aware of the risks and
hazards and will warn my child according to what I feel is appropriate.

Cheer appreciates parents encouraging their children to follow all the safety
rules and the coaches’ instructions. We will endeavor to provide a safe
environment for our students and teach through proper progressions. We
will also keep a small student/teacher ratio to allow a more interactive
learning environment.
